To help us prepare your tax return, please follow the steps below:

1. Please complete this questionnaire fully and accurately.  Your responses will help us ensure your return is prepared correctly and includes all applicable deductions and credits.
2. Collect all documents needed to file your return, including (but not limited to): W-2s and/or 1099s, identification documents (for you and dependents), income and expense records, and any other relevant tax forms.  Please bring these documents with you to your appointment.
3. If you have additional details, explanations, or questions about your tax situation, please use the section labeled Additional Comments.  This section helps us address special circumstances, clarify any unusual items, or prepare for specific concerns during your appointment.
